    Knee Surgery

     

    1. SPORTS SURGERY:

    Treatment of different knee injuries using the latest arthroscopic techniques with minimal scarring and shorter recovery.

     

    -Meniscal injuries.

    These structures are not only a cause of pain and functional limitation, which prevents patients from carrying out normal sports practice, but it is also crucial to preserve them as much as possible since they constitute a "cushion" that prevents wear and tear on the joint and that it frequently triggers premature osteoarthritis in the event of injury or extensive resections. In our unit we perform meniscal sutures and artificial meniscus implants to avoid this while allowing the return to sport as soon as possible with all the guarantees.

     

    -Articular cartilage injuries.

    The latest technology is used to repair these injuries including small implants, microfractures, stem cells or arthroscopy-assisted growth factors, and supportive collagen meshes.

     

    -Injuries to the cruciate ligament.

    A fundamental component of the stability of the knee whose injury usually makes it impossible to carry out sports practice. High quality reconstructions are performed with the least waiting time to resume the sport.

     

    -Instabilities and chondropathy of the patella.

    By means of arthroscopic techniques, the lesions of the patella ligaments are repaired and the cartilaginous defects are repaired. Osteotomies are also performed to correct a faulty alignment of the patella that many times causes this pathology.

     

    2. OSTEOARTHRITIS SURGERY:

    The best surgical techniques that exist today are used for the treatment of the damaged joint. Depending on the degree of injury, more or less aggressive techniques will be used, although our priority is to preserve as much healthy tissue as possible and this is where our success lies.

     

    -Arthroscopic techniques.

    Mainly used to treat joint cartilage injuries when osteoarthritis is very localized.

     

    -Femoral and tibial osteotomies.

    They are performed when there is a deformity associated with osteoarthritis whose correction can improve it. Minimally invasive techniques are used with a very small scar and computerized navigation in order to obtain a correction of maximum precision to improve the results.

     

    -Partial joint prostheses.

    We try to replace only the damaged parts of the joint, thus preserving the maximum bone free of osteoarthritis, being the results are spectacular. We use computer navigation and the incisions are minimal, which allows us an early recovery and a quality and durable prosthesis.

     

    -Total prostheses.

    In the same way we perform complete prosthetic replacements when the joint is completely damaged. We also use computer browsers and minimally invasive surgeries when technically feasible and the deformities are not excessive.

    Sports Medicine

     

    Treatment of chronic tendon injuries.

    These problems, which may seem of little magnitude, are actually disabling for athletes since they prevent in most cases the correct performance of physical exercise.

    Healing is mainly achieved with the following treatments: rehabilitation with eccentric exercises, infiltration with Growth Factors or Platelet Rich Plasma (PRP) and surgery (in serious cases).

     

    Treatment of joint injuries.

    In initial cases, biological treatment may be sufficient (chondroprotectors, viscosupplementation with collagen or hyaluronic acid, infiltration with Growth Factors or PRP and stem cells or MSc). In more severe cases we use arthroscopic treatment (shaving, joint toilette or microperforations and regenerative supplementation) and open surgery, depending on the case.

     

    Treatment of acute muscle injuries.

    Here we also use Growth Factors (PRP) infiltration with very good results, in such a way that the patient can join the sport much earlier than expected.

     

    Foot & Ankle Surgery

     

    -Joint preserving surgery.

    Through surgical intervention of the soft tissues (joint capsule, tendons, etc.) or osteotomies in the bones (cuts to align them), the correction of multiple deformities that may exist in the foot and ankle and that cause pain, are achieved, before osteoarthritis or destruction of the joints occur.

    All this using the latest minimally invasive surgery techniques to minimize complications of conventional open surgery. In this way, hallux valgus (bunions), claw toes, cavus and flat feet, varus and valgus feet, adult foot and ankle deformities, Morton's neuromas, plantar fasciitis and calcaneal spurs, nail injuries, can be corrected. heel pain and Achilles tendonitis and other tendons of the foot, ankle instabilities or chronic pain after repeated sprains, sports injuries, birth deformities, neurological problems, soft tissue injuries or tumors and cysts and infections.

     

    -Surgery on damaged joints.

    Once the joints are partially or totally destroyed, other surgical techniques will be necessary, namely, arthroscopy for cleaning and perforations or reconstruction of the damaged cartilage when possible, arthrodiastasis (a very new technique that consists of placing an external fixator around the foot that distracts the joint for a time and helps to regenerate the cartilage of the joint, greatly decreasing discomfort), prosthetics of both the foot and fingers and the ankle in selected patients and the so-called arthrodesis (fusion of the affected joint only).

     

    -Fractures of the foot and ankle.

    Surgery is performed in all possible locations of these injuries, emphasizing surgery without incision or minimal one, to reduce the probability of complications and accelerate the recovery process.

     

    -Diabetic foot.

    Extensive experience in the treatment of diabetes lesions in the foot and ankle, from prevention (so important in these patients to avoid its consequences) to cure (of ulcers infected or not, neuropathic or Charcot foot and circulatory problems).

     

    Regeneration of damaged tissues

     

    Currently we have regenerative therapies that are giving very good results, already proven, with hardly any side effects and that, in many cases, prevent or delay a surgical intervention, for the benefit of our patients.

     

    For cartilage injuries of the joints and the well-known osteoarthritis, whether due to traumatic or degenerative causes, that is, as a consequence of trauma to the joint or progressive wear and tear, non-invasive therapies have been designed very recently that substantially improves the symptoms, without having to resort to surgery in many cases and, in others, to delay it a lot, with which the results and survival of prostheses, for example, are greater.

     

    For this, Dr. de las Heras Romero uses different last generation compounds, the most innovative on the market.

    One of them is Collagen. It is a natural supplement that is administered directly to the damaged joint through an infiltration that can be of great help in the treatment and prevention of osteoarthritis. It consists of a protein that has been widely used in Aesthetic Medicine for the treatment of wrinkles and deterioration of the skin in general, without any side effects, and which has recently been seen to also play an important role in the repair of defects in the joints, bones, tendons and muscles. It is a fundamental component of tissues, which is why it is assimilated without any problem, exerting a hydration and elasticity effect on them.

     

    Another important substance is called Hyaluronic Acid. It is a molecule that is once again an integral part of the tissues, especially the joints and that has been seen to be lost with age and wear and tear. This is why the infiltration in the different joints of the body has been designed. It is a product with a very viscous texture, made up of large molecules, which exert a repair, lubricant and shock absorbing effect, for example when walking and exercising.

     

    But there are other, even more innovative treatments, which in this case are not laboratory formulas, but are extracted from the body itself.

    One of them is the use of Growth Factors, also called Platelet Rich Plasma. By means of a simple blood extraction, which can be carried out in the same consultation, the blood obtained is processed by centrifugation and supplementation with different compounds, thus isolating and activating only a few molecules that are part of the blood platelets, which are the responsible for the repair of tissues, being able to be used not only in joints but also in damaged bones, tendons and muscles.

     

    Finally mention of what is called Stem Cells. I also use it for the cure of these ailments and they have the potential to generate new cartilage since these cells, which are extracted by aspiration of fat tissue, are the precursors of the different cells that are going to be part of the different tissues, therefore that when injected into damaged tissues they can end up differentiating themselves and generate new tissue that repairs the damaged one. It is being used in multiple pathologies, not only in Traumatology, with promising results.
